Behavioral Health Case Manager
Fast-growing outpatient mental health clinic is seeking individuals to work with adults that have serious mental illness as community-based case managers. Case managers spend approximately 70% of the time in the field teaching consumers the skills necessary to manage their illness and live as independently as possible. Teach and assist them with skills such as:
* Helping consumers identify their personal recovery goals.
* Providing psychoeducation and teaching symptom management skills.
* Accessing community resources such as entitlements, food banks, housing and medical care.
* Teaching coping skills including relaxation skills or anger management.
* Helping consumers develop their personal support systems.
Qualifications:
* BA, MA, MSW or HS with 2yrs adult mental health experience.
* Good driving record.
* Strong organization, communication, computer and time management skills.
