JOB DESCRIPTION:

Qualifications
1. BS or MSW Degree required from accredited educational institution.
2. Licensed in the State of Maryland as a LCSW or LCSW-C.
3. Five (5) years experience in social work with an emphasis in acute care and/or oncology.
4. Ability to communicate and possess strong organization skills
Responsibilities
1.Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients on unit
2.Develop professional knowledge base
3.Participate in care coordination rounds
4.Demonstrate awareness of patient rights including confidentiality, privacy, informed consent, surrogate decision making and advance directives
5.Counsel patients and families around the issues of emotional adjustment to illness, disability, hospitalization and/or discharge plan
6.Demonstrate the knowledge and skills necessary to determine age specific needs
7.Provides information and referral services to patients and families
8.Demonstrate working knowledge of community resources
9. Interacts with patients, families, and health care team to develop proactive plans for continued care and discharge planning
10.Documents assessment and interventions in the medical record
11.Provides support for crisis intervention, bereavement counseling.
12.Intervenes when social/economic issues affect health care and disposition
