POSITION:

Career Transition Specialist

JOB DESCRIPTION:

St. Luke’s House is seeking a fulltime Career Transition Specialist to provide vocational/educational support and case management services for 16-21 year old youths with emotional disabilities. 



Duties include:


-Conducting skills assessment in the areas of work, independent living, family and social skills. 


-Coordinating and advocating with community service providers, schools, and families for necessary supports for a successful transition.


-Providing individualized assistance in identifying transition, career and/or educational goals which includes the following: 


*Career education and career development

*College or vocational education, development and placement 


*Job development and job placement 


*Family support services 


*Identification of supplementary service needs 


*Identification and linkage with community resources. 



Candidates must have a bachelor’s degree and experience working with youth/young adults with serious emotional problems. You must have your own transportation and no more than 2 points on your driving record. Knowledge of the Spanish language is a plus.
